SNI Companies is looking for Registered Nurses to support our client in the Jacksonville, FL area. This is a contract to hire opportunity within their Infectious Disease clinic. The ideal candidate will have an active RN license and possess expectational patient care skills.

Responsible for the care coordination of pediatric, adult and adolescent patients in a multi-disciplinary outpatient medical clinic by providing nurse case management services including: assisting with pre/post-test HIV counseling, managing and scheduling patients for specialty and research care, identifying and updating CDC HIV/AIDS classification, HIV/AIDS reporting, facilitating referrals, educating patients, family and staff, facilitating patient support groups, and maintaining open communication with providers.

Job Summary:
Responsible for the care coordination of pediatric, adult and adolescent patients in a multi-disciplinary outpatient medical clinic by providing nurse case management services including: assisting with pre/post-test HIV counseling, managing and scheduling patients for specialty and research care, identifying and updating CDC HIV/AIDS classification, HIV/AIDS reporting, facilitating referrals, educating patients, family and staff, facilitating patient support groups, and maintaining open communication with providers.
Duties:
- Clinic Care Coordination: Assists the clinician in delivering patient care including taking vital signs, preparing immunizations, Dressing changes, pulse oximetry and respiratory treatments and other required procedures. Accurately and efficiently documents findings, plans and interventions in the medical record. Evaluates the effectiveness of care and documents subsequent changes to plan or follow up on medical record.
- Plan of Care: Assesses individual physical, psychosocial and developmental needs of patients. Preprocesses and post processes the patient chart for each clinic visit. Establishes or reviews plan of care and modifies as needed based on assessment. Implements the established plan of care competently and consistently. Demonstrates advanced clinical and technical skills and can accurately assess and intervene when deviations from expected outcomes occur.
- Provides Patient and Family Education: Identifies specific education needs for patients/families. Accesses and utilizes appropriate teaching tools and techniques to facilitate patient/family education. Utilizes appropriate style and level of communication. Evaluates and documents effectiveness of teaching plan.
- Documentation: Responsible for documenting contacts, chart notes and other required data in medical record and the pertinent information systems used by organization including Immunization Databases, Provide Case Management CMS, Specialty SNAQ Reports, Epic and other systems as required.
- Monitors the quality of practice within HIV infected/exposed patient population by participating in continuous quality improvement program.
- Participates as a member of the healthcare team and acts as liaison between patients, families and the clinical team to ensure quality customer service. Acts as a resource person for peers and inpatient staff and demonstrates clinical expertise for specific population. Attends pertinent educational activities and reviews current literature relevant to clinical area of expertise.
- Under Universal Precautions performs phlebotomy, HIV testing and other assigned labs as necessary.
- Recognizes the need for referral to other sub specialists, other agencies for providers and facilitates the process. Coordinates with outside pharmacies, patient hospital discharge planning and specialists as necessary for patient care.
- Attends and participates in research, department and hospital-wide meetings. Some minimum travel between clinics.
- Active R.N. license in the State of Florida
- Completion of a nationally accredited nursing program
- Active BLS Certification
- 2+ years of patient care required
- 1+ years of nursing experience required
- Infectious Disease/HIV experience preferred
- Outpatient clinical experience preferred
- Ability to perform phlebotomy on pediatric, adult, and geriatric patients
- Able to work with people from diverse ethnic, cultural, socioeconomic and sexual preference backgrounds
