SNI is seeking a Registered Nurse to perform comprehensive home health assessments for Skilled Home Health, Group Adult Foster Care, and Adult Foster Care programs. Responsibilities include conducting OASIS and MDS assessments, health evaluations, coordinating care plans, documenting clinical findings, and supervising allied health team members.
Requirements:

Active MA RN license

1+ year of experience with MDS and OASIS

Reliable transportation
