Pharmacy Care Coordinator

Role:
The Patient Care Coordinator provides communication and handles support for the infusion Pharmacist and the Pharmacy Department. Assists with pharmacy functions including, but not limited to, handling inbound telephone calls and emails, making outbound calls and emails, troubleshooting, packaging of finished products, and other tasks as deemed necessary by the Pharmacy Manager or Pharmacy Coordinator.

Qualifications:

  • High school diploma or GED
  • Licensed Pharmacy Technician preferred
  • Previous healthcare experience and medical terminology preferred
  • Ability to handle physical workload involved and stress of working in hospice care
  • Ability to work independently and prioritize multiple tasks under minimal direction
  • Strong communication (verbal and written) skills, including face-to-face, telephone, and email
  • Proficient at data entry and Microsoft Outlook, Word and Excel; strong level of computer literacy
  • Must be able to interact successfully with patients, families, and other agencies while presenting a positive and professional image
  • Able to demonstrate strong ethical base related to mission, including confidentiality, sensitivity, and flexibility

Job Responsibilities:

  • Reviews and prioritizes prescription information using pharmacy software resources for Pharmacists and Pharmacy Technicians to process.
  • Answers all incoming telephone calls and screens for appropriate routing or handling of calls.
  • Reviews and responds to emails from clinical staff regarding requests that do not require clinical or medical intervention.
  • Completes patient demographic data entry for maintaining up-to-date medication profiles.
  • Provides support for the infusion Pharmacists and Pharmacy Department.
  • Coordinates delivery of finished infusions, packages infusion supplies and makes appropriate outbound calls.
  • Assists with the sorting and packaging of finished prescription products.
  • Packages and prepares prescriptions for UPS delivery.
  • Checks for outdated medications each month and removes outdated medications from inventory.
  • Performs other duties as assigned.
