Patient Care Coordinator (Temporary) possible TTH
Job Summary:

  • Responsible for verifying all hospitalized patient invoices are complete/correct and under the appropriate attending doctor.
  • Assisting nurses/doctors in patient updates and relaying messages as needed.
  • Communicating with referring veterinarians regarding referrals or updates as needed.
  • Primary contact for pharmacies which includes communicating the specifics of prescriptions and facilitating client needs.
  • Answer and return client calls/emails regarding a variety of questions/concerns, which will include close communication with doctors to relay medical information as appropriate.
  • Point of contact for communicating interdepartmentally regarding communication and care of clients/patients.
  • Assist nurses/doctors in entering charges or going over previously discussed estimates in person/over the phone as needed.
  • Must document all communication in Woofware, our electronic medical record keeping system.
  • Responsible for verifying all lab work sent out through the ICU department was submitted correctly and received by the laboratory.
    • Monitors status of return and ensure client is contacted with results in a timely fashion.
  • Responsible for generating and conducting patient 'check in' calls, daily, for patients who have previously discharged from the CritiCare/ICU department.
  • Must be comfortable assisting with patients on the medical floor as needed- i.e restraining for various procedures.
  • May be asked to assist with filling medications or going over discharge instructions with client taking their pet home.
  • May be asked to help with keeping stock of department inventory.
  • Must be comfortable going over financial aspect of cases with clients and discussing end of life care/arrangements.
  • Must be calm, patient and empathetic with clients/co-workers in a high-stress, fast paced environment.
  • This position has the potential to evolve to meet the department's needs.
